Practice Based Commissioning

Comments: Unless we get Practice Based Commissioing (or something similar), underpinning, and acting as a counterbalance to, Payment by Results (National Tariff) the NHS is heading for a bigger overspend than ever before. I work daily with General Practitioners who are struggling to implement the changes required, often blocked by a PCT who don't want them to succeed. A clear vision of the way forward is required, then step back and let GP's / Primary Care implement, but make it a supported implementation of innovative policies driven, from the bottom up, by GP's and Primary Care Healthcare Professionals, who understand what is required. All to often policies are handed down and imposed with little or no consultation, and / or support.  We have 152 PCTs all implementing Practice Based Commissioning in different ways!!  It isn't right and badly needs sorted or we will watse £millions more in the process. I would be delighted to support policy development within Primary Care to make it meaningful and lasting.
